- Tuesdays 8:00 am-12:00 pm and 1:00 pm-4:00 pm
- Immunizations by appointment only
- Call (419)399-3921 to schedule; please have insurance information ready if applicable
What to Bring
- Any previous immunization records
- All insurance cards, if applicable
- If the Child is under 18, they must be accompanied by an adult. If that adult is someone other than the parent or guardian of the child, written permission is required. Contact the health department for more information
Immunization Fees
- We bill most insurances, including Medicaid
- Cost assistance programs are available for those who do not have insurance or do not have vaccine coverage. Call PCHD for more information
